WE SERVE CHILDREN AND ADOLESCENTS (AGES 5-20): INTENSIVE IN-HOME

The Intensive In-Home service focuses on the mental health needs of the child and provides intensive, time-limited interventions provided typically but not solely in the residence of a child who is at risk of being moved into an out-of-home placement or who is being transitioned to home from an out-of-home placement due to documented clinical needs of the child. The service includes the following:

  • Individual and family counseling
  • Coordination with other required services
  • Facilitating the transition to home from out-of-home placement
  • Training to increase appropriate communication skills
  • Counseling to understand and practice appropriate problem-solving, anger management, and interpersonal skills

The goals of the service include:

  1. Providing structured, outcome-driven therapeutic treatment, which improves the child’s level of functioning in the school, home, and community.
  2. Building trusting relationships with families to promote their positive involvement in the school, community, and society.
  3. Developing and implementing an individualized service plan (ISP) that promotes the healthy development of the client in order to discharge the individual from services as a positive, productive member of society.
  4. Identifying and promoting the strength of the individual and family to build family stability and promote improved functioning.
  5. Providing the necessary linkage of supports through service coordination with doctors, schools, courts, hospitals, Family Assessment and Planning Teams (FAPT), social services, parents and others involved in the client’s treatment in order to provide a comprehensive, holistic approach to service.

WE SERVE ADULTS AND 16 YEARS OR OLDER ADOLESCENTS: MENTAL HEALTH SKILL-BUILDING

The Mental Health Skill-Building service provides goal-directed training to enable adult individuals to achieve and maintain community stability and independence in the most appropriate, least restrictive environment. The service includes goal-directed training in the following areas:

  • Functional skills and appropriate behavior related to the individual’s health and safety
  • Activities of daily living
  • Use of community resources
  • Assistance with adherence to medication prescriptions
  • Monitoring health, nutrition, and physical condition

The goals of the service include:

  1. Providing structured, outcome driven treatment, which strives to increase the adult client’s level of self-sufficiency and independence within their homes or the least restrictive environments.
  2. Building trusting relationships with adult clients, while establishing collaborative relationships with other agencies and providers who are relatively involved in their care.
  3. Developing and implementing an individualized service plan (ISP) that provides a guide for the client’s treatment and subsequent advancement towards goal achievement.
  4. Identifying and promoting each client from a strengths-based approach beginning with their individualized assessment, which gauges their level of need, personal challenges, areas of strength and functional abilities.
  5. Providing the necessary linkage of supports through service coordination with resource agencies and increased exposure to valuable resources within the community that will ultimately assist them towards greater personal management.
